- If using the pdf: fill it out electronically and print, or print and fill it out by hand. Bring the completed form to your appointment.
Financial Policy
- Download the form.
- Fill out the form electronically, then print and sign. OR print the form and fill it out by hand.
- Bring a printed copy of your completed form to your appointment.
*If you do not have access to a computer, you will need to arrive 30 minutes before your appointment so that you can complete the forms.
